Test Report Forms
A sample IELTS for UKVI test report is shown below next to a standard IELTS test report. The 
standard test report is not suitable for visa applications if you are coming to study on a pre-degree programme. 
Correct Form 
The image below shows what an IELTS for UKVI test report form looks like. It contains a UKVI Number 
underneath the Candidate ID, and a CEFR level (circled in red). Other features include: 

Module type is indicated in the top right-hand corner. It should read "Academic". 

Centre number is indicated in the top left-hand corner. It should be one of the 
UKVI approved secure 
English language test centres
. 

Test Report Form Number is indicated in the bottom right-hand corner. Please provide this number and 
the UKVI reference number to the BISC via the online portal

Incorrect Form 

The image below shows what a standard IELTS test report form looks like. This is NOT an IELTS for 
UKVI test report form. Note it does not have a UKVI Number underneath the Candidate ID.
