Patient Registration & Reporting Instructions

Accompanying each lens is a patient identification card (to be given to the patient after 

implantation). The patient should be advised to carry this card at all times. A postcard and 8 

stickers are also provided. One sticker should be placed on the patient ID card, one on the 

postcard and the others are to be used for the doctor’s records. Please return the postcard 

with the sticker to USIOL.

Adverse events/complaints that may reasonably be regarded as lens-related should be 

reported to U.S. IOL 2500 Sandersville Rd., Lexington, KY 40511, USA or email: [e-mail].

Warranty

 

Lens is void of all warranties expressed or implied if:

■

 Lens is resterilized by customer

■

 Lens is repackaged by customer

■

 Lens is altered in any manner
